How a Data Entry Program Runs
Most data quality problems are instruction problems. The first two steps prevent them.

What Bad Data Actually Costs, and Where It Hides
Data entry is often bought on a rate per record, and compared on that basis alone. The rate is the smallest part of the cost. A mis-keyed invoice is paid twice or not at all. A wrong address sends a shipment, a statement or a legal notice to the wrong place. A duplicated customer record splits their history, so service agents see half the picture and marketing mails them twice. Each of these costs more to find and fix than the original entry cost to make.
The errors that do the damage are rarely random typing mistakes; those are caught by validation. They are interpretation errors — an ambiguous date format, a handwritten figure, a field left blank on the source, a product that fits two categories. They repeat, because the same person makes the same reasonable guess every time. That is why a written rule for every ambiguity, and a route for escalating the ones no rule covers, does more for quality than any amount of checking afterwards.
It also explains why the cheapest rate is often the most expensive service. A provider paid purely per record has every incentive to guess and move on. How do you measure accuracy?
